When ordering a schedule II controlled substance from McKesson Medical-Surgical, a corresponding DEA 222 form is required to be submitted before we fulfill the order, unless you are utilizingMcKesson CSOSManager℠.

McKesson Medical-Surgical customers’ DEA 222 forms must be submitted to our Jacksonville, FL location.

As of February 4, 2022, McKesson Medical-Surgical customers serviced out of the Sacramento, CA controlled drug warehouse were consolidated into the Memphis, TN controlled drug warehouse.

Discontinuation of triplicate DEA Form 222

As of October 31, 2021, the Drug Enforcement Administration has mandated the use of single sheet DEA 222 forms for those providers who procure schedule I and II controlled substances.

At this time, triplicate DEA 222 forms are no longer available or allowed to be utilized, according to theCode of Federal Regulations.
